Case Management services are provided to assist the client in accessing health and psychosocial services in a timely and coordinated manner; and to promote continuity of care and maximize independence through government, private, and community resources.
Case management services are available to eligible clients who are:
- living with HIV
- survivors of sexual assault, same-sex domestic violence or a bias/hate crime
- at-risk GLBT and questioning youth
- GLBT seniors age 60+
